Anatomy – This is an extremely uncommon anomaly with the RCA originating from the proximal Circumflex.  The vessel courses behind the aorta in a path that is reminiscent of the more common anomalous LCx origin from the RC Cusp. The distal RCA is not well seen in the two views available, but other shots confirm acute distal RCA occlusion at the origin of the PDA.
